Liver Cirrhosis

Liver cirrhosis affects an estimated 2.2 million adults in the United States, with age-adjusted mortality increasing from 14.9 to 21.9 per 100,000 between 2010 and 2021. In a 2018 insurance-based analysis, cirrhosis prevalence among Medicare Advantage enrollees reached 1.67%, higher than other insurance groups examined. In a separate cohort, older adults newly diagnosed with cirrhosis had higher all-cause mortality than younger patients (HR 2.26). More than half of patients 65 and older with cirrhosis have two or more additional chronic conditions, such as diabetes, cardiovascular disease, or chronic kidney disease. The disease trajectory is stark: median survival exceeds 12 years in compensated cirrhosis but falls below 1.5 years once decompensation —ascites, portal-hypertensive gastrointestinal bleeding, or hepatic encephalopathy— occurs.
